New Enrolments
We are pleased to advise we can offer enrolment with Dr Kate Richardson or Dr Andrew Lott if you live in the Waitemata DHB Region. Please complete this online application form link for each patient enrolling.
Each patient 16 and over will need to complete and sign their own enrolment form.
The following things are required in order to move forward with the application:

NHI number – if unknown please contact your previous GP for this

NZ Passport OR
-
NZ Birth Certificate and NZ Driver’s Licence OR
-
Overseas Passport and valid Visa
*If enrolling a newborn, please use a parent passport as ID until the birth certificate is available
*If enrolling a child under 16 with no photo ID just a birth certificate is acceptable
When received, we will let you know if there is anything else required, and let you know when your enrolment is confirmed. Please allow 5 - 10 business days to process your enrolment before booking an appointment.

Telehealth Appointments
If you are unable to get an appointment with us, we would recommend contacting PocketLab or CareHQ. Both are staffed by New Zealand GPs.

Holiday Plan
We will be closing at 3pm on 24th December 2025, reopening on Monday 5th January 2026.
We will not be processing any prescription requests (including voicemails or emails) during this time, and portal prescription requests will be deactivated.
If you need medical attention over the break, your options include:

Healthline is a free service to call for health advice and information on 0800 611 116.

Virtual appointments are available through CareHQ via your Indici patient portal. Southern Cross members can book through your MySouthernCross app.

PocketLab is another great telehealth service available for any health concerns over the break.

For more urgent or in person care, you can go to North Shore or Waitakere Hospital, or White Cross Henderson at 131 Lincoln Road, which is a 24/7 walk-in clinic.
Script Requests
If you need a repeat prescription, please use the patient portal, fill out our Nurse Contact form online, or leave a phone message detailing what medications you need.
If you cannot come into the clinic to collect your script, we can send an e-prescription to your pharmacy of choice. If you require this service, please specify this in your message.
