New Enrolments

We are pleased to advise we can offer enrolment with Dr Kate Richardson or Dr Andrew Lott if you live in the Waitemata DHB Region.  Please complete this online application form link for each patient enrolling.  

 

Each patient 16 and over will need to complete and sign their own enrolment form.  

 

The following things are required in order to move forward with the application: 

 

  • NHI number – if unknown please contact your previous GP for this

  • NZ Passport  OR

  • NZ Birth Certificate and NZ Driver’s Licence   OR

  • Overseas Passport and valid Visa

 

*If enrolling a newborn, please use a parent passport as ID until the birth certificate is available

*If enrolling a child under 16 with no photo ID just a birth certificate is acceptable

 

When received, we will let you know if there is anything else required, and let you know when your enrolment is confirmed. Please allow 5 - 10 business days to process your enrolment before booking an appointment.

 

Practice fees are available on the services page of our website. This includes the new patient fee payable for your first consultation. ​​

Telehealth Appointments

If you are unable to get an appointment with us, we would recommend contacting PocketLab or CareHQ. Both are staffed by New Zealand GPs.

Script Requests

If you need a repeat prescription, please use the patient portal, fill out our Nurse Contact form online, or leave a phone message detailing what medications you need.

 

If you cannot come into the clinic to collect your script, we can send an e-prescription to your pharmacy of choice. If you require this service, please specify this in your message.
