New patient enrolments are currently on hold (as at 25th November 2020) due to overwhelming demand. We will be reviewing our capacity to take on new patients on a regular basis. Please monitor this website for updates.
If you are a family member of an existing patient please email reception at and we will send you out an enrolment form.

If you need a prescription request, please fill out our Nurse Contact form online, or leave a phone message detailing what medications you need.
If you cannot come into the clinic to collect your script, we can fax and post the script to your pharmacy of choice. If you require this service, please specify this in your message. Note that faxing/posting a prescription does incur an additional $5 charge.
If you have more than 2 weeks supply of medication, please only request a repeat of your medications when you are in your final 2 weeks of supply.
